    Those are the questions.

    1-size of enclosure
2-# on inhabitants - specifically ( if there is another frog ---size differences ?)
    3-humidity
    4-temp
5-water - type - for both misting and soaking dish
    6-materials used for substrate
7-enclosure set up i.e. plants( live or artificial) wood, bark etc -how were things prepared prior to being put into the viv
    8-main food source
    9-vitamins and calcium ? ( how often )
    10-lighting
11-what is, specifically, being used to maintain the temperature of the enclosure
12-when is the last time he ate
    13-have you found poop lately
14-a pic would be great ( including the frog ) any little cell phone pic is fine
15- how old is the frog
16-how long have you owned him
17- is the frog wild caught or captive bred
18- frog food- how often and if its diverse what other feeders are used as treats
19- about how often the frog is handled
20-is the enclosure is kept in a high or low traffic area

If I have left anything out, please feel free to chime in !

Save one animal and it doesn't change the world, but it surely changes the world for that one animal!

    1-size of enclosure 16 x 16 x 20
2-# on inhabitants - specifically ( if there is another frog ---size differences ?) he was alone
3-humidity I don't remember the #s for this and question 4; however, they were what they were supposed to be b/c I monitored the thermometer and hygrometer often. I knew at the time what the humdity and temperature where supposed to be.
    4-temp
5-water - type - for both misting and soaking dish distilled water
6-materials used for substrate coconut husk
7-enclosure set up i.e. plants( live or artificial) wood, bark etc -how were things prepared prior to being put into the viv - live plants, and I used the Exo-Terra Jungle Vine for all his climbing. I would just have opened the packages and put them in the terrarium.
    8-main food source live crickets
9-vitamins and calcium ? ( how often )I don't remember any vitamins or calcium. I would have put calcium on the crickets he ate.
    10-lighting a UV bulb
11-what is, specifically, being used to maintain the temperature of the enclosure I also used a basking bulb that gave off heat. If the temperature starting rising, I would remove it.
12-when is the last time he ate he had eaten the night before
13-have you found poop lately there was poop in his terrarium when he died. i would clean it out weekly.
14-a pic would be great ( including the frog ) any little cell phone pic is fine my avatar is a picture of him. I have other pictures too.
15- how old is the frog He was about 15 months old when he died
16-how long have you owned him I owned him for one year when he died
17- is the frog wild caught or captive breed captive bred; I bought him at a Repticon show
18- frog food- how often and if its diverse what other feeders are used as treat At the time he died he was eating every other day. He only ate crickets. However, if there was some type of work he ate, I would have given him some of those too from time to time. He died over a year ago, so it is hard for me to remembers these details.
19- about how often the frog is handled I probably only handled him for the picture in the avatar
20-is the enclosure is kept in a high or low traffic area my kitchen snack bar, with me in the area all the time.

These are all around the same time. The one on the right is the same as my avatar but I thought it might come out clearer. I see I did have some wood -- it would be oak and I would have soaked it in very hot water before I used it.
