    The skin sac you're describing sounds like brumation... But all your husbandry is pretty spot-on so I don't know why he'd be doing it. The good news is it does sound like brumation rather than an illness so you don't have to immediately worry.
    How long have you had this frog, and has it always done this?

    Someone more experienced in breeding may be able to recommend a way to bring him out by simulating the beginning of the breeding season, but since your temps/humidity are already where they should be I can't think of any other way to bring him out.

    Your humidity is a little high, but that shouldn't cause brumation. Only being a year old and constantly attempting to go into dormancy is a little odd. Does the frog have a water dish? If not place a shallow water dish in the enclosure.

    He's had a dish since I set up the tank. Haven't seen him use it though.

    Many rarely will, but if you bathe the frog in its enclosure where it feels more secure it may help.

    Because I suspect he's blind, I've added an LED light to his tank during the day. I hope this stimulates his day/night cycle so he doesn't try to brumate so often.
