What is wrong with this common pond frog??

Hiya not sure if I'm doing this right as I'm new.
I went out to my garden today and found one of our gorgeous domestic pond frogs was sprawled out on a lily pad looking very sorry for its self, I can see three spots that look like they're missing skin (only small and isn't deep) so assumed a cat/fox had played with it, but I've also noticed it's very skinny and extremely weak, it may be old as I think it's been coming to our pond for a long time now (can tell by the the markings) but is there anything else it could be? And is there anything I can do to help?
I tried to attach photos but my phone won't let me.

From what you describe, I would suggest gently catching up the frog and getting it to an interested vet, who will be able to assess it properly and treat as appropriate. Not sure where you are, but if you look online or call your local vets and see who in the area they generally refer amphibs to you should be able to find a vet willing to treat him.
