On the pinky mice, once or twice a month maybe? I breed Dubia so thats usually what I use, but I have mealies and supers occasionally for my bearded dragons so I gave them those sometimes for variety. They were decent sized, about 2-2.5 inches

I had the exact same problems. First I got a baby, but she ended up dying in the exact way you described. So I got an adult male instead, but same thing. I couldn't figure out what the problem was, so I just stopped keeping pacman frogs.
