it is probably from lack of air movement in the tank. you could remove it, add microfauna, or open the tank for a while each day to allow air transfer. longer than you normally would. an air circulation fan in the tank would also be effective, but a bit more complex than the other options.

The tank has a screen top. Could air circulation still be a problem? I'll get a picture tonight.
