Large enough tank,

Water dechlorinator, the ones for fish work.

Substrate
(cocoa husk ground finely or plantation soil,
or paper towels changed every couple of days,
or if you want a water tank, a few inches of water with things to climb out onto. But you're going to need a small filter and you're going to have to look at water PH.

Something to climb on that's fairly sturdy. Not grapevine, that grows mold

Places to hide: artificial plants they can hide behind, hollow cork bark etc.

Lighting: UV is probably not really necessary unless you want to grow plants (although UV may help frogs a little it's not clear).

Supplements: multivitamin, Calcium with D3 for sure

Water bowl if the tank doesn't have water at the bottom

Thermometer and hygrometer

Heat source: depends on how cold or hot your house gets. You can get reptitherm heat patches to go on the side of the tank. That doesn't do much for the overall temperature, just gives the frog a place to bask. Or you could use a ceramic heat-emitting bulb in a dome light over the tank. A 60 watt works on mine. Heat lights are okay too, but some frogs seem to get annoyed by red ones left on at night. And don't use them directly over something plastic that could melt.
