    Hello Matt,
    I would email Dr Frye. ( email a photo to him )
    In the meantime, continue the neosporin. ( WITHOUT pain reliever)
    ? put him into a QT tank ----especially if he has a tank mate.
    Scrub and change the soaking dish every day. hot hot water and friction !!!!! no soap

    Most importantly....this looks like a tare/puncture ? Inspect the tank for share objects.
    ie Any fake plants ? sharp ends or tips on decor or branches ?

    His enclosure should be warmer " Day: 29 °C (85 °F); Night: 20 - 24 °C (68 - 75 °F) "

    Not sure if maintaining 85 daytime is necessary -- but 80F is ...... for proper digestion

    Feed him well and warm him up a bit ...he will heal more quickly

    Thank you for the advise. I'll everything immediately and keep everyone posted.

    Regarding his enclosure: There are no other animals other than the naturally occurring bug-mite-things that come out from wet coco fiber.
