Hello, thats a beautyfull toad you got there, I especially love those spiny warts

As it is a tropical species it needs somewhat higher temperatures in the daytime. I would try to reach at least 25°C at day, with room temperature at night.
For the swollen bodyparts I have no idea what could be the cause. Just hope its nothing serious... but as he eats and behaves normally he probably doesnt suffer much.
