I got a blue samurai pacman last week. He was about the size of a silver dollar when I got him. He ate a night crawler the day after I got him, and another 2 days later. But after the second worm he buried himself and now I haven't seen him in 2 days (well he might be coming out when I sleep but I don't think so) I just don't know if I should be worried.
1. 10 gallon
10G is too big and will stress out frog until about 3.5-4 in. snout to vent (SVL) length. You can get a smaller (i.e. 5 gal) size enclosure or use a smaller plastic critter keeper inside the 10G.
3. Between 75-80
Need to get temps to 84F day with a 5-10 degree drop at night. You can use a lamp (red bulbs or ceramic heat emitters in a dimmer type dome) heat source in combo with a pad mounted on the side. Can also cover most of top screen with aluminum foil to help keep humidity and heat in.
5. Soaking dish with filtered water and misted with filtered water.
Filtered water can still include chloramines and some filtering systems are very aggressive to include removing needed minerals. Recommend using treated tap (Seachem Prime works well) for both frog's bath and mixing with shredded coco. Since Pacman's bury and they absorb water through belly skin; it's very important substrate has no toxic chlorine, so recommend replacing all of it now.
9. Reptivite every other feeding

10. Natural light, I have a heat lamp that I use a bit in the early morning or night when it's really cold but I only leave it on when I can keep an eye on it so it doesn't make it too hot.
You can control heat lamps with controllers, either manual dimmers (rheostats) or auto thermostats.
