Recommend get a hygrometer (digital are better than analog). Anything between 50-65% is OK.

This is a bit too low. Day should have a basking area of 30C and night temps drop to 24C or so. A heat mat or heat tape might be required if room is cold.

What kind of "fresh water?" Recommend use dechlorinated tap (Seachem Prime is good dechlorinator).

If these are under water or too large for frog to eat it's OK. If there are small and frog could grab one when lounging for food; it could cause an impaction.

9. Vitamins and calcium? (how often) - Calcium every now and then
Frogs in captivity need food supplements. CA/D3 and vitamins should be offered; specially when feeding crickets. 10. Lighting - 24w compact UV
UV comes in several kinds and in the higher type (i.e. UV 10) might be too much. You will need to adjust this and/or replace with day bulb that produces heat (see no. 4 above).

11. What is being used to maintain the temperature of the enclosure - Currently nothing.

19. How often the frog is handled - Every few days
20. Is the enclosure kept in a high or low traffic area - high
Should wash hands well and do a final rinse with dechlorinated tap before handling frog. Also, recommend covering frog enclosure sides and back with any paper you like to reduce stress.
