Hello and welcome to FF ! How big was Clyde (snout to vent length - SVL) when you got it and how big is frog now? 4-5 small earthworms weekly sound like too little to me.

Frog poop loks like a log, big by frog size standard. If in water dish you will know, it stinks bad. Sometimes they poop in their hole and bury it with substrate. Time to search with tweezers is when they are out or in soaking bath. Look for harder pieces that are darker than shredded coco. Spot cleaning like this will help keep substrate cleaner until monthly changes.

A fat frog is fat all over and when touched feels hard like your skin. A bloated frog will feel squishy when touched and edema fluid will actually displace as you touch. If bloated it's a serious condition, usually caused by chronic low temperatures. A puffed frog will fill itself with air when stressed or feels threatened to look bigger and meaner.
