I would add, question regarding water parameters (NH3/NO2/NO3) to this questionnaire because I would wager a lot of ACF/ADF deaths are a result of tanks high with ammonia/nitrite readings, thus causing the animal to stress and allow opportunistic infection to set in.
High levels of nitrate due to lack of proper maintenance causing stress/infection is probably right up there too.
Also, "does your tank have a proper lid to prevent escape" is another good once. Many frogs are lost due to escape and dehydration.
I'm sure there's some other good questions for this as well.
