This poor baby needs a lot of love.

First thing I would do is to setup a hospital for it.
Moist paper towels for substrate. Water dish and heat source.

A soak in solution of de chlorinated water with 10% unflavored pedolite warmed to 80F

And bactine to treat the open sores.

And a visit to the vet asap.

Agreed, this little guy has been through the wars. We need to hop into action.

Louis has it right on, although some plants would reduce stress for the little guy, and a lid so you don't have a frog escapee once he's healed.

You can either buy an amphibian/reptile water conditioner, or Reverse osmosis water for the 1/10 unflavored pedialyte - 9/10 dechlorinated water soak. Regular tap water, if untreated, may kill him.

For this, like Louis said, he'll need a trip to the Exotic Vet ASAP.

I would suggest feeding him some earthworms, either store-bought or from a pesticide/fertilizer/additive free compost/garden. They're so much more nutritious than crickets, and easier to digest too. You can chop them into smaller chunks (ugh, I know) if he can't eat the full worm.

Fingers crossed for the little guy.
    My Amphibians:
1.0.0 Rana Catesbiana (Bumpy Digtoad )
    1.0.0 Pseudacris Regilla (Levi )
1.1.0 Ambystoma Macrodactylum (Urtham and Gargan )
2.2.0 Bombina Orientalis ( Rosa, Sasha, Aleksis, and Dimitri )

Rest in Peace, Gnag the Nameless, Chrome, and Thermidor

I have some Nightcrawers, so i cut some up for him. My wife was telling me he was dry when she got him from the store. We are looking for Exotic Pet Vets in our area. Is there a site that will have some listing to help us find one.

You can ask your local pet store about a vet as well, I asked who they've used. They recommended my Vet who did all she could for my Frog.
Good Luck, Hope He/She has a quick recovery.
