Have you tested your water, bad water quality is the root of most problems? What size tank? Is this frog housed with other fish/frogs? Is there anything in the tank that can hurt the frog? Sharp objects? Need more information.
I agree with everything said here. We need to know what your water ammonia and nitrite readings are, temperature, tank mates, tank decor before we can give sound advice.
The diet you are feeding though is horrible. It would be like you eating McDonalds fries and candy all the time. I know a few other people from overseas have mentioned not being able to find Reptomin food sticks, but really any aquatic reptile or amphibian pellet is better than what your currently feeding.
