Yes, Kingcam is right on. You need to get your frog some Panacur from a vet. Call your vet and see if he'll test a stool sample for you. This can be done without a checkup. If he will prescribe the Panacur for you without a visit, that may be a good start. Dust his food with the powder one day a week for 4 weeks. Panacur is a dewormer.
If he is eating and thin this is most likely why.
It is a must to remove the substrate and clean thoroughly. I'd use a hospital tank for 5 weeks. 4 weeks of treatment and one week after to monitor.
