I'm John Clare, in Ohio (the guy who "owns" FrogForum). The computer I do all my work on (including FF and Caudata.org) died last weekend and I need to sell these frogs to pay for a new one.

Shipping is by shipyourreptiles' FedEx service only. I've finally sworn off USPS after they drop kicked a box of Phyllobates terribilis. My zip is 44135.

Regarding live arrival guarantee. You ship frogs enough and you will eventually have a couple of problems. I pack well and I guarantee live arrival. My concern is for the actual frogs arriving alive. That's what a live arrival guarantee means to me. You can have every cent of your money back if the frogs are dead on arrival no matter when it is. Money is the last thing I care about if the frogs are dead or hurt. The only caveat to that is if you do not answer when the carrier makes the scheduled delivery, in which case you assume responsibility.
