Do I need a vet?

My tomatoe frog has a sore on his back it is three bumps on his back. I thought he was just getting to big for the little home I gave him so it took it out immediately and got him a bigger one. Well the sores are not going away. Further more around his eyes and nose is white and it looks like the tops of his eye lids are not shaped right. Then again I'm always worried my frogs are sick. Look at his eyes....and the brown spot on his back. I've been changing his water constantly and spraying his back with water as well.
