Daylight hiding is understandable. I figure most animals will hide from predators. It is evening swimming I am wondering about. You might consider taking her out of the tank and putting her in a clean de-chlorinated bucket just to get her apart if she is sick. Make sure to give her a hiding place there too. Maybe take some pictures. See if she has a white coating to her. Anything that might help people here know what is happening. Is she bloated? Maybe she is ready to drop eggs?
It is best to get the liquid test kits rather than the dip sticks. More accurate. When was the last time you did a water change and how much?
There are few things that will plague this froggers. I have had some good and bad luck but overall they are pretty straight forward creatures.
