It could have been either, but my guess would be that he died due to an infection from the prolapsed intestine in the other thread. Frogs rarely recover from that sort of situation.
Worms basically eat particles in the soil, so if an earthworm has been in soil with fertilizer sometimes washing it may not get all of the toxins off, since there may be some left in its digestive system. If you can, go by a bait store or pet store and buy a pack of worms from there for your turtle. You can also get freeze dried or pellet turtle food from pet stores or off the internet if you are having a hard time finding live stuff; it's not great, but it might be safer.
If you have a plastic bin with a cover, it's really easy and cheap to breed your own worms using ones from your yard to start it off. You keep mentioning a lot of different reptiles/amphibians that you have, so you may want to look into doing that. That way you know they're safe and don't have to worry.
