Okay, so to answer the questions I have been posed:

Right-o on the vet. However, I live in bum-f-corn-town-Illinois, so amphibian vets are few and far between. I have to drive a couple of hours for my Jackson's Chameleon to be cared for as well.

Your frogs day time temp needs to be 80 to 85 degrees F you need to stay within this range during the day. No 70s. At nigt temps can be 75 to 79 degrees F. Preferably kept at 78. Humidity levels must be 70 to 80% and maintained.

--I apologize, I was not clear in my original post. Falstaff stays right around 80 degrees during the day, we drop him to 75 at night. Thus the range. My humidity is exactly between the level you mention above.

Are you using de-chlorinated water or spring water? I ask this because you must.

--We get properly treated water from the local pet store for exorbitant prices for all of our pets.

Never and I mean never have rocks in your Pacman's enclosure unless they are so big that there is no possible way for your frog to swallow them. They can be eateb by accident causong impaction(severe constipation) which can cause death.

--Falstaff's rocks are quite large, I am aware of the dangers too-small substrate can pose.

Do you use de-chlorinated water or spring water to expand your frogs substrate?

-- I use the same treated water from the pet store for all purposes, including rinsing/cleaning/hydrating/humidifying. We live in a town with a lot of minerals in the water so I don't want to risk injury or sickness with my pets.

When was the last time your frog pooped? Was it runny or normal?

--It was in July a week or so after he ate the mouse, and it was normal.

Do you dust your Pacman's food with calcium and a multivitamin?

--I have in the past, but as he is not eating, it doesn't do much good.
