When I had to take Grif to the vet, I put her in a critter keeper with damp paper towels. Your frog might feel more secure with some dirt and hides, but it does make it harder for the vet to check the frog if it is covered in dirt. You'll also want to watch the any hides don't shift while you're driving. The heat is going to be your bigger problem. Those don't hold heat too well without a heat source and its going to be no match for the cold winter air. A big temperature drop could send your frog into shock. Warm your car up good before taking him out. If you have a heat pack that would definitely be good. Try wrapping a towel around the carrier to help insulate it. Vet trips are stressful not matter what, but try to him and yourself calm. I hope everything goes well and your little guy gets better soon!
