Hopper was a WTF and he had a prolapse and he passed away. What causes this and is there anything I could have done? I feel terrible.

Could be a few different things, how were you keeping him?

Trachycephalus resinifictrix - Trachycephalus nigromaculatus - Agalychnis callidryas - Agalychnis spurelli - Phyllomedusa sauvagii - Phyllomedusa bicolor - Phyllomedusa vaillanti - Phyllomedusa tomopterna - Gastrotheca riobambae - Anotheca spinosa - Cruziohyla craspedopus - Cruziohyla calcarifer - Hyla arborea - Litoria caerulea.
He was in a 35 gallon tank with Eco earth substrate. He had a pond with filtered spring water. He was fed crickets. He had one other tank mate. I had him for a year and he was fine.

I'm sorry.
If your other frog ever has problems it might be a good idea to move it to another tank with paper towels on the bottom. You can try applying sugar or honey to the prolapse to try to reduce swelling. Keep the prolapse damp. You might even want to try gently pushing it back in with a moistened Q-tip if nothing else works.
But having said all that, sometimes nothing will work. It's just one of those things.
I did try a sugar bath in a quarantine tank but it didn't help. I just want to know if something caused it so I can change what ever it might be.

Sometimes if a frog has a protozoan overload it can irritate them and cause prolapse. Or at least I've heard. If a frog is impacted and straining that can be a problem. Otherwise, sometimes prolapses just happen like hernias in humans.
