It sounds like bloat, you will want to start him on antibiotics soon
"Bloat" is a sign, not a diagnosis. It can arise from all sorts of causes such as liver problems, kidney problems, cardiovascular problems, internal infection with all sorts of different bugs, tumour, etc. Antibiotics may be appropriate but may well not be (especially if this has been going on for a long time; bacterial infection in particular would generally have got a lot better or a lot worse in that time). If at all possible you really need to get him to a vet for attempts at diagnosis, or at least a more rational treatment regime. Throwing antibiotics at an undiagnosed problem is not a good idea - quite apart from potential ineffectiveness and potential side effects, irresponsible use of antibiotics is potentially leading us towards severe human medical complications (google "antibiotic apocalypse").
